1. Streamline Your Check-In Process

You can download and complete your new patient paperwork in advance.

2. Please Bring with You:

  • A list of all of your current medications including the dosage and number of times you take them daily.  

  • Your insurance card, if insured

  • A current photo ID (for patients 18 years and older)

  • The name, phone, and fax number of your family physician
